The whole family is sick anyway! Doesn't anybody remember an episode of their reality TV show when this tape Kim had made when she was a teenager mysteriously emerged that someone was trying to blackmail her with and she had a total meltdown on the show and their mother brought in the FBI and everything to squash the situation? Well it's because it was a tape with her and her other sister in it doing inappropriate things together they all have issues and lacked basic parental guidance when they were young.

Yeah, it's probably some incest going on or has in the past. The family seems to have some major issues. Kim is a plastic surgery addict after all.
